The caller never waited. Had this rung out, most people would have dialed the next company on the list rather than left a message, and you would not know the job existed.
9:15pm, a question you have answered a hundred times

- Recognized as clinical, not scheduling
- Followed your after-hours rule: transferred to the duty number
- Told the caller a person was being connected
This is the call that matters most. The assistant did not offer an opinion about swelling, because you never gave it one and it will not invent one. It recognized the call was outside what it handles and moved it to a person.
